TEKNIK PEMERIKSAAN OSSA MANUS DENGAN KASUS POST ORIF PHALANX DEXTRA DI INSTALASI RADIOLOGI RUMAH SAKIT X

Abstract

 Pemeriksaan radiologi merupakan salah satu pemeriksaan penunjang dalam menegakkan diagnosa fraktur post orif pada os manus. Untuk melihat kelainan pada manus menggunakan proyeksi posteroanterior (PA), oblique dan lateral, untuk melihat fraktur dan dislokasi menggunakan proyeksi posteroanterior (PA) dan oblique, sedangkan untuk melihat adanya corpus alineum menggunakan proyeksi posteroanterior (PA) dan oblique. Penelitian ini bertujuan untuk mengetahui teknik dan prosedur pelaksanaan pemeriksaan ossa manus dengan kasus post orif phalanx dextra.Metode penelitian yang digunakan yaitu penelitian deskriptif dengan pendekatan studi kasus. Hasil penelitian yang didapatkan yaitu proyeksi posteroanterior (PA) dan oblique, bisa terlihat fraktur shaft phalanx media digiti IV manus dextra yang terpasang internal fixator (pinning), dengan posisi baik, callus (+), trabekulasi tulang normal, celah dan permukaan sendi tampak baik, tidak tampak erosi/destruksi tulang, tidak tampak soft tissue mass/swelling. Kesimpulan dari penelitian ini yaitu, pada pasien dengan klinis post orif phalanx dextra dilakukan dengan 2 proyeksi yaitu posteroanterior (PA) dan oblique karena sudah cukup memberikan gambaran komprehensif dua dimensi yang menyilang, memungkinkan dokter untuk memverifikasi posisi implan (plat dan sekrup) serta tanpa persiapan khusus pasien hanya dianjurkan untuk melepas benda-benda yang dapat menimbulkan bayangan radiopaque.

ABSTRACT

Radiological examination serves as a diagnostic aid in assessing fractures following Open Reduction Internal Fixation (ORIF) of the hand bones. According to posteroanterior (PA), oblique, and lateral projections are used to visualize abnormalities in the hand; PA and oblique projections are used to assess fractures and dislocations; and PA and oblique projections are used to detect foreign bodies. This study aimed to determine the techniques and procedures for examining the hand bones in a case of post-ORIF right phalangeal fracture. A descriptive research method with a case study approach was employed. The results showed that on PA and oblique projections, a fracture of the shaft of the middle phalanx of the right fourth digit was visible; an internal fixation device (pinning) was in place with good positioning, callus formation was present, bone trabeculation appeared normal, joint spaces and surfaces looked intact, and there was no evidence of bone erosion/destruction or soft tissue masses/swelling. The study concludes that for patients with a history of ORIF for a right phalangeal fracture, a two-projection protocol—PA and oblique—is sufficient to provide a comprehensive, orthogonal two-dimensional view, allowing the physician to verify implant (plate and screw) positioning; furthermore, no special patient preparation is required other than the removal of objects that could create radiopaque artifacts.
